*SOCIAL WORKER *
SCHOOL WELLNESS PROGRAM _*General Summary*_ Under the supervision of the Director of Personal and Community Health, the Social Worker works with the School Wellness Program in the local School district. The Social Worker will be assessing social and emotional needs and identifying risk areas that could lead to unhealthy outcomes for adolescents. Assists students in obtaining assistance to meet those needs. Provides direct counseling services and psychosocial support to adolescents. The mental health services provided shall meet the recognized, current standards of practice for care and treatment of children and/or adolescents. _*Essential Functions *__(An employee in this position may be called upon to do any or all the following functions. These examples do not include all of the tasks that the employee may be expected to perform)_ 1. Masters prepared Social Worker licensed in the State of Michigan to work with a Registered Nurse in the local School Wellness Program. 2. The Social Worker will provide full-time services during the school year in no more than two school buildings. 3. Follows the approved work plan for the School Wellness Program and all approved policies and procedures. 4. Assesses social and emotional factors related to the adolescents illness, need for care, response to treatment, adjustment to care for progress. 5. Develops individual service plans based on input from the student, staff and members of the students family, if applicable. Monitors implementation of service plans conferring with nurses and other staff as necessary and provides follow up and assessment of services in relation to goals and objectives of the service plan. 6. Makes biopsychosocial assessments, treatment plan, and discharge summaries of the students needs including their strengths and weaknesses and progress or lack thereof towards a positive outcome. 7. Functions as a case manager for designated students, which includes the coordination between various agencies, medical provider and the student. Provides comprehensive care management and coordinates services provided through a multidisciplinary approach collaborating extensively with multiple community agencies. 8. Networks and maintains good working relations with other community agencies and advocates for the students by identifying the needs of individuals and making referrals to community and service agencies. Represents the health department at community functions and meetings. 9. Documents all student contact, maintains case records and completes reports as required by department policy and procedures and the requirements of third-party funding sources. 10. Other duties as assigned. _*Employment Qualifications*_ *Education:* Masters Degree in Social Work. *Experience:* Two years of professional social work experience providing services to families with intense needs. *Other Requirements:* Possession of a valid State of Michigan drivers license.
