Weekend On-Call Triage LPN – Make a Difference in Just One Weekend!
SummitWest Care – Grand Junction, CO
Saturday 8:00 AM – Monday 8:00 AM | $700 Weekend Stipend + $25–$30/hr DOE

Are you a Licensed Practical Nurse (LPN) looking for meaningful work with flexibility and purpose? SummitWest Care , a mission-driven non-profit home health agency in Grand Junction , is seeking a Weekend On-Call Triage LPN to join our compassionate team.

This vital role allows you to make a real impact while enjoying a compact schedule – work one weekend and help patients get the care they need, when they need it most .

About the Role:

  • Be on-call from Saturday at 8:00 AM through Monday at 8:00 AM

  • Respond to patient calls and care concerns over the phone

  • Provide in-person field visits when necessary

  • Collaborate with the care team to assess and resolve urgent home health needs

  • Accurately document triage decisions and patient visits

Compensation & Perks:

  • $700 flat stipend for the full on-call weekend

  • $25–$30/hour for any time spent on visits (DOE)

  • Mileage reimbursement for patient visits

  • Work from home when not in the field!

What We're Looking For:

  • Active LPN license in the State of Colorado

  • Home health experience preferred

  • Excellent judgment and problem-solving under pressure

  • Ability to prioritize and communicate effectively

  • Reliable transportation, valid driver's license, and insurance

  • Comfortable working independently in a home health setting
