What You’ll Do:

As a Care Team Assistant II , you’ll play a vital role in coordinating care and supporting our clinical team. Your responsibilities will include:

• Receiving and processing home care referrals
• Scheduling and rescheduling visits based on patient needs and staff availability
• Acting as a communication hub between physicians, case managers, staff, and clients
• Educating inquirers about our services and home care options
• Collaborating with payroll to ensure accurate time records
• Conducting client admissions/evaluations (excluding PHC program clients)
• Submitting timely reports and documentation

What We’re Looking For:

• High school diploma or GED (required)
• 6 months to 1 year of experience in a home health or medical setting (preferred)
• Willingness to travel for client visits/hourly care
• Strong organizational and communication skills
• Bilingual in Spanish (preferred)
• Home health experience (strongly preferred)
